個人資料收集聲明（PICS）

  1. 參加者／潛在客戶可自願提供與此活動相關的所需個人資料。未能提供所需個人資料可能導致參加者／潛在客戶無法參與此活動。

  2. On-us Group Holdings Ltd (「On-us」)可能不時收集此活動及市場推廣的參加者／潛在客戶的個人資料。 On-us轉移本人的個人資料至永明香港。個人資料（即電郵地址）將被 永明香港使用、及被轉移至永明香港的理財顧問／金融中介機構／服務供應商及被其使用，我們及他們可能使用參加者／潛在客戶的個人資料，以電郵方法與他／她聯絡，處理在聯絡過程中可能傳達的任何跟進活動及作換領禮品通知以及要收集的其他個人資料。

  3. 提供本人的個人資料予永明香港及其安排之理財顧問用作直銷推廣用途（包括並不限於以電郵方式聯絡本人）。相關產品和服務受限於永明香港的條款及規則及私隱政策。 永明香港將所有收集的個人資料，資料將於本活動結束後12個月內銷毀。
  1. 任何參加者／潛在客戶有權要求查閱及更正其個人資料或拒絕接收來自永明香港的市場資訊。參加者／潛在客 戶應將有關要求以電郵形式發送至 [email protected]

 

Personal Information Collection Statement (PICS)

  1. Participants/potential Clients may voluntarily provide personal information necessary for this activity. Failure to provide the required personal information may result in the participant/potential Client being unable to participate in this activity.

  2. On-us Group Holdings Ltd ("On-us") may collect personal information from participants/potential Clients of this activity and marketing from time to time. On-us transfers my personal information to Sun Life Hong Kong. Personal information (i.e., email address) will be used by Sun Life Hong Kong and transferred to and used by Sun Life Hong Kong's advisors/financial intermediaries/service providers. We and they may use the participant's/potential Client's personal information to contact him/her via email, process any follow-up activities that may be communicated during contact, notify about gift redemption, and collect other personal information as required.

  3. My personal information is provided to Sun Life Hong Kong and its arranged advisors for direct marketing purposes (including but not limited to contacting me via email). The relevant products and services are subject to Sun Life Hong Kong's terms and conditions and privacy policy. Sun Life Hong Kong will destroy all collected personal information within 12 months after the end of this activity.

  4. Any participant/potential Client has the right to request access to and correction of their personal information or to refuse to receive marketing information from Sun Life Hong Kong. The participant / prospective Client should make his/her request by email at [email protected].
